Chronic Disease Self-Management Program

The Chronic Disease Self-Management Program (CDSMP), was developed at Stanford University and introduced to the Antelope Valley by HDMG’s Associate Medical Director, Dr. Don Parazo.

Subjects covered include:  1) techniques to deal with problems such as frustration, fatigue, pain and isolation; 2) appropriate exercise; 3) appropriate use of medications; 4) communicating effectively; 5) nutrition; and 6) how to evaluate treatments.

The process in which the program is taught is what makes it effective. Classes are highly participative, where mutual support and success build the participants’ confidence in their ability to manage their health and maintain active and fulfilling lives.

The program is free and is open to anyone who suffers from chronic diseases, as well as those who care for someone with a chronic disease, such as diabetes, arthritis, asthma, congestive heart failure and depression, just to name a few.
