Senior Wellness Center

The HDMG Senior Wellness Center is a brand new facility designed to provide the best in wellness programs and services free of charge, exclusively for HDMG patients who are Medicare Advantage members and Medicare Beneficiaries. When you join the HDMG Senior Wellness Club you can access all the services and benefits available at the Wellness Center, including: a one hour thorough health exam and assessment appointment only,-by free health screenings, health education workshops, fitness sessions and lots of fun activities.

To join, simply complete a Senior Wellness Club Application Form and hand it to one of our receptionists. You'll receive a new member packet, membership card and a club t-shirt.

We're open Monday-Friday from 8:30 a.m. to 5:00 p.m., and are located behind our main facility at 43779 N. 15th Street West, Lancaster, CA 93534. For more information call 661-951-3050.
