A program designed to offer a Coordinator which will assist the Caring for Life Case Manager in assuring all medical needs and questions are addressed.   A Communication for Life Coordinator will communicate via telephone on a scheduled basis to ask simple questions regarding the patients health status. Their primary focus is on promoting patient health and wellness.

No referral is necessary.
