High Desert Medical Group employs a team of social services professionals to assists patients and families with complex social situations. This team consists of Licensed Clinical Social Workers (LCSW) and social services designees (SSD). Our LCSWs assist with front line counseling, urgent mental health concerns and substance abuse matters. Our SSDs work with patients and families in a variety of settings to assist with placement, advanced care planning and a variety of other social concerns. The mission of the HDMG social services team is to help enhance the members well-being, and to assist in solving problems related to complex needs that many of our patient’s experience. Please call 661-945-5984 should you need assistance.
